Complete an application for each student and family member who would like to receive care at the Longfellow Family Clinic.
Lakeshore Community Health Care partnered with the Sheboygan Area School District to offer medical care for students and family members at Longfellow Elementary School.
By providing care in school, the Longfellow Family Clinic eliminates important barriers to care. Students and their families do not have to travel for medical care, so students are able to stay in school, parents do not have to leave work, and transportation is not needed.

- Immunizations/Vaccines
- Flu Shots
- Physical Exams for School or Work
- Acute Care (Sore throat, cough, flu, etc.)
- Specialty Care Referrals, as needed
